Job Description:
Description Position Overview: Describes the overall purpose of
the position or why the position exists The Social Work Coordinator
provides oversight, training, workflow development, and support for
Social Workers to ensure patients and their families gain access to
community resources, including services for substance use,
psychiatric, medical, social, perinatal resources, residential,
financial, educational, and other services appropriate for the
patient. The Social Work Coordinator shares responsibility for the
operational integrity of Social Work services to ensure timely,
high quality integrated care. Essential Duties and
Responsibilities: The following description is a general
representation of the essential duties and responsibilities of the
position. Provide oversight of Social Workers, including training,
evaluating, onboarding, and guiding on best practices. Develop,
monitor, and maintain efficient Social Work workflows with EMR in
collaboration with Behavioral Health Leadership. Assess patient
needs and provide care coordination for linkage to appropriate
services to enhance patient health and well-being, improve patient
satisfaction, and reduce overall health care cost. Coordinate with
internal sources and external community partners to ensure care
coordination for appropriate services as needed. Track referrals
for patient compliance and assist in reducing any barriers patient
may face in receiving needed services as needed. Monitor adherence
to care plans, evaluate effectiveness, monitor patient progress to
identify successes and areas for improvement. Maintain accurate and
thorough documentation on patience services, in accordance with
organizational standards and requirements. Support with
identifying, develop[ping, and maintaining new and existing
community partnerships, formal and informal support systems, and
community building opportunities to help patients access additional
support systems. Partner with Community Health Worker/HUB
Coordinator, Referral Coordinator, Medical, Behavioral Health, and
other internal providers to ensure continuity of patient access and
referrals for resource needs. Provide consultation as requested.
Identify opportunities for quality improvement and provide
recommendations with supporting data to Behavioral Health
Leadership. Support NFP’s Mission, Vision, and Values in the
strategic direction of social work services, facilitate change,
motivate staff, and improve processes through effective
problem-solving. Maintain a working knowledge of applicable
policies and procedures to ensure compliance and safeguard the
delivery of quality social work services. Support with managing
crisis calls that come into NFP of a Behavioral Health clinical
nature as needed. Other duties as assigned. Qualifications
Education, Licensure, Experience: Master’s Degree in Social Work
required Current, Active, and Unsretricted Social Work License
(LSW) in the State of Ohio required. Independent licensure (LISW,
LISW-S) preferred. Current BLS/CPR certification 3-5 years work
experience in Social Work, Care Coordination, Case Management, or
similar professional role required. Knowledge and Skills:
Knowledgeable of community resources and social services landscape
in the Greater Cleveland area. Experience coordinating with
external community partners to meet patient needs. Computer
proficiency in Microsoft Office Suite, EPIC EMR/EHR, database
management systems. Able to communicate effectively with the
appropriate level of cultural sensitivity. Able to effectively
manage a patient population with diverse needs and acuity. Ability
to speak a relevant language other than English fluently preferred.
Essential Physical Demands and Working Environment: Able to
communicate effectively, in writing and orally, including through
the use of electronic communication devices. Primarily office work
environment, but able to be flexible with changing patient and work
environment needs. Able to sit, stand, walk, and stoop for extended
periods of time. Occasional lifting of up to 25 lbs. Willingness to
travel locally and between sites. Active Driver’s License and
Automobile Insurance preferred. The physical demands and work
